1. Purpose of This Consent
This document explains how the Medical Track uses telehealth — the use of electronic communications and information technology to deliver health-related services without an in-person visit. By acknowledging this consent during intake, you confirm that you understand the nature of telehealth services and the limitations and risks involved.
2. Who Provides Your Care
OS reboot is a wellness program partner. OS reboot does not employ physicians, does not operate a pharmacy, and does not make medical decisions.
If you elect the Medical Track, your medical intake is reviewed by an independent licensed physician practicing through an independent licensed Management Services Organization (MSO) (the "Medical Entity") that contracts with the prescribing medical practice. All clinical decisions are made by the prescribing physician based on their independent professional judgment.
If a prescription is written, it is dispensed by an independent state-licensed 503A compounding pharmacy contracted through the Medical Entity. OS reboot does not handle, store, or dispense any medication.
3. Nature of Telehealth Services
Telehealth services available through the Medical Track may include:
- Asynchronous review of your written intake by the prescribing physician;
- Secure messaging through the Wellpep platform or other communication channels;
- Synchronous video or phone consultations, if requested by the physician or required by state law;
- Electronic prescribing where clinically appropriate;
- Remote monitoring and follow-up.
The specific modality used will be determined by the prescribing physician based on your medical needs, your state of residence, and applicable law.
4. Benefits of Telehealth
- Convenience — access to a licensed physician without in-person travel.
- Privacy — care delivered in your own home or chosen location.
- Continuity — ongoing communication with the care team through a secure platform.
- Access — care may be available in geographies or schedules where in-person care is limited.
5. Risks and Limitations of Telehealth
You acknowledge that telehealth has inherent risks and limitations, including but not limited to:
- No physical examination. The physician cannot perform a hands-on physical exam, which may limit the information available for a diagnosis or treatment decision.
- Possible technology failures. Equipment failure, software issues, internet outages, or other technical problems may interrupt or delay care.
- Security risks. While we use industry-standard security measures, electronic transmission of information carries inherent risks of unauthorized access, interception, or disclosure. We mitigate these through encryption and access controls, but no system is 100% secure.
- Incomplete information. The physician relies on the accuracy and completeness of the information you provide. Incomplete or inaccurate information may affect the quality of clinical decisions.
- Limitations on certain conditions. Some medical conditions cannot be appropriately evaluated or treated by telehealth and require in-person care. The physician may decline to proceed if telehealth is not appropriate for your situation.
- State law restrictions. Telehealth services are subject to the laws of the state where you are located at the time of the encounter. Not all services are available in all states.
6. Compounded Medications
If the physician determines that a compounded medication is clinically appropriate, the medication will be dispensed by a state-licensed 503A compounding pharmacy. You acknowledge that:
- Compounded medications are not FDA-approved. The FDA does not evaluate compounded medications for safety, effectiveness, or quality prior to use. This is a regulatory distinction; safety derives from the prescriber's review and the pharmacy's sterile-compounding standards under USP <797>.
- Compounded medications are patient-specific. Each prescription is compounded individually based on the physician's order.
- Some active ingredients used in compounded medications may have FDA-approved brand-name versions, but the compounded versions themselves are not FDA-approved.
- Compounded medications cannot be returned once dispensed. See our Refund Policy.
7. Possible Risks of Treatment
All medications carry risks. The physician will discuss specific risks relevant to your treatment during their review. Common categories of risk include:
- Side effects (mild, moderate, or in rare cases, severe);
- Allergic reactions;
- Drug interactions;
- Conditions that worsen rather than improve;
- The need for additional treatment or follow-up.
You are responsible for promptly reporting symptoms, side effects, or concerns to the care team through the channels provided. For emergency symptoms — including chest pain, trouble breathing, severe allergic reactions, or signs of a stroke — call 911 immediately and seek emergency care.
8. Your Responsibilities
By electing the Medical Track and consenting to telehealth services, you agree to:
- Provide truthful, accurate, and complete information during intake and ongoing communications;
- Disclose all current medications, allergies, medical conditions, and relevant medical history;
- Follow the prescribed care plan, including dosing instructions, follow-ups, and lab orders;
- Promptly report symptoms, side effects, or concerns;
- Use the medication only as prescribed and not share, resell, or transfer it;
- Maintain the confidentiality of your account credentials;
- Notify the care team if you move to a different state or country, as this may affect the physician's ability to continue care.
9. Right to Withdraw Consent
You may withdraw consent for telehealth services at any time by notifying the care team in writing through the channels provided. Withdrawing consent will end the telehealth relationship but will not affect any treatment already provided.
Withdrawal of consent does not entitle you to a refund of fees paid for services already rendered (such as the $20 physician review fee where a prescription has been written). See Refund Policy.
10. Continuity of Care
You acknowledge that telehealth services through the Medical Track do not replace a relationship with a primary care physician. We strongly recommend that you maintain a primary care relationship for routine and emergency care, and that you share information about your Medical Track participation with your primary care provider.
11. Records and Privacy
Records of your telehealth encounters, including your intake, communications, and prescriptions, are maintained by the medical services organization and the prescribing physician's practice in accordance with applicable laws and HIPAA. Records may be accessed, copied, or transmitted as permitted by law and as described in the medical group's Notice of Privacy Practices and our HIPAA Notice of Privacy Practices.
12. Acknowledgment
By acknowledging this consent during the intake process, you confirm that:
- You have read and understood this Telehealth Informed Consent;
- You have had the opportunity to ask questions;
- You voluntarily consent to receive telehealth services through the Medical Track;
- You understand the risks, benefits, alternatives, and limitations of telehealth;
- You understand that OS reboot is a wellness program partner and that all medical decisions are made by independent licensed physicians;
- You are 18 years of age or older;
- You are physically located in a U.S. state where telehealth services are authorized by the prescribing physician's license.
If you do not agree to these terms, do not proceed with the Medical Track intake. You can continue to use the free Daily Protocol and wellness coaching features without consenting to telehealth services.
